We also provide advice and guidance in notifying regulatory agencies and being available to assist clients in preparation for and during investigations by inspectors.

The incident investigation process we employ entails:

  • Determining the sequence of events that resulted in the incident.
  • Conducting impartial interviews with workers and witness to establishing key events that are crucial to understanding what happened.
  • Identifying the causes with contributed directly or indirectly to the incident.
  • Identifying the deficiencies with the management systems that allowed the incident to occur.
  • Recommending corrective actions to prevent a recurrence and to remedy the root causes.
